Information this directory collects
Provider and facility pages use public directory, practice, VA-associated, and NPI source records. When you submit a form, the directory collects the contact and request information you choose to provide, such as your name, email, phone, ZIP code, organization, care category, selected provider, and referral status.
Do not submit Social Security numbers, medical records, diagnoses, or detailed medical information. This directory is not a medical record system or emergency service.
Funnel and attribution data
The site records limited first-party events such as page views, searches, provider selections, form opens, form starts, completed lead forms, submission errors, and click-to-call actions. Event records may include an anonymous browser identifier, a session identifier, page type, provider ID or NPI, care category, referring website, and UTM campaign fields.
The analytics endpoint does not intentionally store IP addresses, free-text symptoms, form field contents, or precise search locations. The browser uses local storage for an anonymous identifier and first-touch campaign attribution, and session storage for a session identifier.
How information is used
Lead information is used to respond to requests, route inquiries, support provider listing reviews, and improve directory operations. Lead submissions may be stored in Supabase and mirrored to GoHighLevel for follow-up. Hosting and operational notifications may use Vercel and configured email or webhook services.
The directory does not sell personal information. Information may be disclosed when required by law, to protect the service, or to service providers supporting the functions described above.

You can browse public listings without submitting a lead form. You can clear local or session storage through your browser. To request access, correction, or deletion of information you submitted, use the contact page.
